COVID-19 Reporting
As of June 13, 2023, PCC has sunset its vaccination requirement.
How can I report a suspected case or positive test result to the Potomac Curling Club?
- Option 1: Report using our online reporting form (This is the easiest way to help protect our community from the spread of COVID-19)
- Option 2: E-mail covid19report@curldc.org
Reporting is voluntary, but telling us about a suspected case or positive test result is helpful to prevent the spread of COVID-19 in our community. It allows us to notify all members and guests who were at the club that they may need to take extra precautions.
If you are making a report via email, the following information will best help us determine who should take additional precautions.
- Your Name
- Phone Number
- Is this a suspected case or a positive test result?
- Date of the positive test
- Date of the onset of symptoms (if any)
- When were you are the Potomac Curling Club? (going back 4-5 days from the positive test or onset of symptoms)
- The names of any potential close contacts
- Individuals playing on the same sheet as you
- Anyone who you were within 6 feeet for more then 15 minutes during a single day at Potomac
Your name, email, phone, and other details will NOT be published.. Notification to our members and guests will only include the timeframe that you were at Potomac.
Example: The Potomac Curling Club has been informed that an individual who was at the club on DATE during TIME has tested positive for COVID-19.
We deeply appreciate you providing as much information as possible so we can make an accurate assessment of who needs to be notified.
If you need to make an anonymous report, you may do so, but it may limit the effectiveness of your report in preventing the spread of COVID-19.
